ABSTRACT: OBJECTIVE: To report the experience of implementing a discharge plan for pregnant and puerperal women with chronic kidney disease assisted at a university hospital. MATERIALS AND METHODS: This is a descriptive study with a qualitative approach of the experience report type. The report was experienced at the University Hospital linked to the Federal University of Maranhão. The action plan was developed through the practical activities of the specialization course in Nephrology from July 2017 to February 2018. RESULTS AND DISCUSSION: The development of a discharge plan associated with a folder with guidelines on chronic kidney disease, with emphasis on self-care practices, was an extremely valuable action for the execution of a more qualified and holistic assistance to these patients. FINAL CONSIDERATIONS: With the proposal to discuss with the team the cases that arrive at the hospital, it was possible to improve the knowledge about the pathology and its complications and, consequently, develop them with these patients, making the learning process more qualified, since these measures were strategies for understanding the importance of treatment and adherence to healthy lifestyle habits by pregnant and postpartum women with chronic kidney disease.
