ABSTRACT: OBJECTIVE: To analyze the repercussions of the Pilates Method in elderly people, with emphasis on balance, functionality, and quality of life. MATERIALS AND METHODS: Narrative literature review in the PubMed, Google Scholar, and Scientific Electronic Library Online databases, using the keywords " Pilates" and " Elderly/Elderly," combined with AND/OR, including studies published between 2012 and 2025, in Portuguese, English, or Spanish, and excluding reviews, meta-analyses, and grey literature. 33 studies were included. RESULTS AND DISCUSSION: Regular practice of the Pilates Method was associated with a reduction in body fat, improvement in anthropometric measurements and laboratory parameters, postural re-education, increased balance, strength, stability, gait cadence, and autonomy in activities of daily living, in addition to possible benefits on mental health. FINAL CONSIDERATIONS: The inclusion of the Pilates Method indicates potential benefits for the quality of life of elderly people.
