ABSTRACT: OBJECTIVE: To analyze the challenges and strategies of nursing care in the face of congenital malformations in neonatal intensive care units. MATERIALS AND METHODS: This is a narrative literature review, conducted in the Virtual Health Library (VHL), Scientific Electronic Library Online (SciELO), and PubMed/MEDLINE databases. Articles published between 2015 and 2025 were considered, a period of greater visibility of malformations associated with congenital Zika virus infection. Seventy-six studies were found, of which nine studies comprised the final sample. RESULTS AND DISCUSSION: The main challenges faced by nursing include the high technical complexity of care for newborns with congenital malformations, emotional barriers, and difficulties in communication with family members. The most frequently described strategies involved the Nursing Care Systematization, individualized care planning, the use of practices aligned with available scientific literature, humanization actions, and continuous emotional support for families in the NICU. FINAL CONSIDERATIONS: The nurse' s role is to ensure qualified neonatal care. The combination of technical and scientific knowledge strengthens the bond with the family and contributes to better clinical outcomes. Continuous training and the adoption of evidence-based protocols are essential to meet the challenges of this complex context.
