ABSTRACT: OBJECTIVE: To describe the prevalence of alterations identified in Biological Neonatal Screening, as well as the coverage and period of sample collection in newborns from a municipality in Maranhão, Brazil. MATERIAL AND METHODS: This is a quantitative, descriptive, and cross-sectional retrospective study, conducted using secondary data provided by the Primary Care Coordination of the city of Imperatriz, Maranhão, Brazil. The study population consisted of live newborns residing in Imperatriz who underwent Biological Neonatal Screening in the municipal public network between 2015 and 2020. The prevalence of diseases screened by the heel prick test was calculated. The study was approved by the Research Ethics Committee of the Federal University of Maranhão, with opinion number 5.127.262. RESULTS: The prevalence of screened diseases was 2.95:10,000 live births. A total of 10,189 newborns were screened, with an annual average of 1,698. Based on this, the coverage rate for the analyzed period was 33.46%. Regarding the detected alterations, there were 535 samples, corresponding to 5.25% of those screened, with the highest prevalence being hemoglobinopathies. CONCLUSION: Low coverage by Primary Care of live births to resident mothers was identified. Among the pathologies, hemoglobinopathies, biotinidase deficiency, and cystic fibrosis predominated.
