ABSTRACT: OBJECTIVE: To synthesize the scientific evidence regarding Non-Communicable Chronic Conditions (NCDs) in Primary Health Care (PHC), focusing on care, service organization, and teamwork. METHOD: A narrative review complemented by a bibliometric word co-occurrence analysis using VOSviewer software. The search was conducted in the Latin American and Caribbean Health Sciences Literature, Scientific Electronic Library Online, CAPES Periodicals Portal, and Google Scholar databases, comprising a final corpus of 19 primary studies. RESULTS: The bibliometric network revealed five interdependent clusters (epidemiological, care/organizational, community, educational, and structural), all centralized in PHC. A dichotomy was observed between the PHC framework and the operational reality, which is frequently marked by care fragmentation, a lack of supplies, and prescriptive biomedical approaches. In contrast, matrix support, task-shifting, and emancipatory health education interventions stood out as powerful strategies to qualify management. FINAL CONSIDERATIONS: PHC is consolidated as a fundamental axis for coordinating care for NCDs. However, systemic resoluteness requires overcoming the curative model in favor of integrated care networks that value multiprofessional work, participatory education, and the sociocultural specificities of the territory.
