ABSTRACT: OBJECTIVE: To critically analyze, based on the scientific literature, the relationships between environmental contaminant exposure, neuroinflammatory mechanisms, and neuropsychiatric outcomes, discussing evidence on the possible influence of these exposures on psychotropic drug response, under the One Health perspective. METHOD: A critical narrative literature review, with searches in PubMed/MEDLINE, Scopus, and Web of Science (2000–2026), which retrieved 25,488 records; 17 studies composed the analysis, organized into five analytical dimensions: contaminant class, exposure pathway, vulnerability window, neurobiological mechanism, and neuropsychiatric/pharmacological outcome. RESULTS: Air pollution showed the most consistent epidemiological evidence, associated with depression, anxiety, and suicide, with residual socioeconomic confounding. Oxidative stress, glial activation, mitochondrial dysfunction, and blood-brain barrier impairment were convergent but nonspecific mechanisms. The perinatal and geriatric windows stood out as periods of greatest vulnerability, the latter due to the convergence of cumulative environmental burden, inflammaging, and reduced cytochrome P450 activity. CYP1A2 induction by polycyclic aromatic hydrocarbons, altering clozapine and olanzapine pharmacokinetics, was the most consolidated mechanistic example of pharmacological implication. FINAL CONSIDERATIONS: Consistent scientific grounds exist for investigating the environment-neurobiology interaction, with varying levels of support across epidemiological association, mechanistic plausibility, and pharmacological implication, reinforcing the need for interdisciplinary research under the One Health perspective.
